Evening and weekend appointments are also

Evenings/Weekends:
An on-call physician may be reached by calling the office
telephone number. The answering service will assist you.
Convenient Location:
Easy access from the Vestal Parkway. Plenty of parking with easy
access to our office. Our facility is a street-level building which
provides easy access for our patients in wheelchairs or with
disabilities.
In Case of Emergency:
Go to the nearest hospital emergency department if you have chest
pains, difficulty breathing, serious injury or bleeding, severe
pain, or are pregnant and experiencing contractions.
Our Staff
Associates in Family Medicine (AFM) is a group of four family
physicians and highly trained clerical and clinical office staff
who provide a full range of services to our patients. We
treat patients of all ages and our practice is accepting new
patients. Same day sick calls are available for established
patients.
Office Telephones
Professional staff are available to take your calls Monday - Friday from 8:30AM - 4:45 PM and Saturday 8:00 AM - Noon.
Lab Services
Lab services are available Monday through
Friday from 7:30 AM to 3:30 PM. No appointment is
necessary. Please bring your physician's order with you. Our lab fax number is 607-321-2456.
Prescription Refills
All refills must be requested 24 hours in advance, Monday
through Thursday. Refills will be called into the pharmacy late in
the afternoon. You can also request a prescription
renewal online.
Missed Appointments
Your appointment is important. If you cannot meet your
appointment please call our office to reschedule at least 24 hours
in advance of your appointment.
Reminders
- Insurance cards must be presented at the time of visit.
- Co-payments are expected at the time of visit.
- Information about financial
assistance is available by calling 607-798-5506 or
607-798-5229.

Tayyebeh N. Borogerdi, FNP
Tayyebeh graduated from SUNY Stony Brook and received her FNP certification from SUNY Binghamton. In addition to English, Tayyebeh speaks Farsi, French, and Spanish.

Dr. Dean graduated from Universidad Autonoma de Guadalajara and SUNY Upstate Health Science Center. He is board certified in Family Practice.

Dr. Malkin graduated from Michigan State University. He is board certified in Family Practice.

Dr. Sy-Vinluan graduated from St. Louis College of Medicine. She is board certified in Family Practice.

Dr. Welch graduated from New York College of Osteopathic Medicine in Old Westbury, NY. He did his residency at United Health Services in Johnson City, NY. Dr. Welch specializes in Family Medicine. In addition to English, he is fluent in Spanish.
